If you are a new patient to Dernancourt Medical Centre, please make note of the following:

For your first appointment, bring your photo ID and a valid Medicare card if you have one (check expiry date)

 

Please allow 5 to 10 minutes before your appointment to fill in a new patient form or Alternatively, you can pre-print the new patient form below:

If you intend on having a procedure done (e.g. excisions/stitches/vaccinations/ IUD insertion) please bring a form of payment.

While we do bulk-bill concession card holders for most consultations, certain procedures and vaccines will incur a private fee.

 

If you are a new work cover patient; you will need to inform your employer prior to the consultation and bring your employer’s contact details for verification. You will be responsible for the full consultation fee until we get confirmation that your employer will be paying for the consultations. These will be billed at the current Work Cover rate
